In your initial visit please provide the following:

A comprehensive medical and dental history

List of medications you are currently taking
- If you had any X-Rays done in the last year please have them emailed to us at office@myblossomdental.com prior your visit.

If you have insurance, please bring any forms or insurance benefit cards (Both dental and medical) with you to the appointment.

All patients under 18 must be accompanied by a parent or guardian at the consultation appointment.

You may pre-register with our office by clicking the Patient Forms button to the right. After you have completed the forms, please make sure to press the Submit button. Your information will automatically be entered into our system so you will not have to fill them again during your initial visit. All information collected online is encrypted to secure your privacy.
Insurance:
We accept most PPO insurance plans. Please provide us with your insurance cards (both dental and medical) and leave the rest to us, our experienced staff will call your insurance company and process your claim for you.
Payment Options:
We offer multiple payment options for you to have the flexibility when paying for our dental services.
We accept cash, personal checks, all major credit cards and we work with Patient Fi, Care Credit and Lending Club Independent Financing Options.)
No matter what your financial or insurance situation, we will do our best to accommodate your individual needs.
